Job Title:                     Healthy Lifestyles Activities Coordinator

Department:                Clinic

Hourly Rate:               $12.00/hr         Full Time – 40 Hours

Exempt:                       No/Hourly

Supervisor:                  Healthy Lifestyles Project Manager, Assistant Clinic Administrator

Summary: The Healthy Lifestyles Activities Coordinator reports to the Healthy Lifestyles Project Manager and has the overall responsibility to plan and coordinate project activities based upon the Healthy Lifestyles Project Manager’s activity plan and direction.

Essential Duties and Responsibilities include the following.

  • Planning and implementation of physical activities including but not limited to; basketball, volleyball, running, walking, football, wrestling, soccer, t-ball, flag football.
  • Teach physical activity, diabetes prevention and diabetes education to youth up to age 19years of age.
  • Work to include cultural components into the activities
  • Participation in the organization of all activities within the project
  • Plan and develop a weekly/monthly activity plan based on the Project Managers direction
  • Participate in the organization and planning of the walking/running club; will conduct weigh-ins for youth participating in the project
  • Attend project planning and weekly staff meetings
  • Assists with gardening project activities
  • Assist with planning and the implementation of special projects events such as; field trips, youth Olympics, awareness activities, and cultural events
  • Assist with pre-school physical fitness/exercise project
  • Submit monthly reports to the Program Manager, Assistant Clinic Administrator and Health Board
  • Maintenance of the weight room and Community Center facilities
  • Assist with Transportation to activities
  • Other duties assigned by supervisor as they relate to the position.

Education and/or Experience:

Required: HS Diploma or equivalent is required, degree in physical education, youth, children and family services or related field preferred.

Three years work experience is required in lieu of post-secondary education.

Must be dependable, self-motivated and able to work independently; ability to work flexible hours such as nights and weekends is required.

Applicant must have ability to maintain confidentiality pursuant to State and Federal requirements as reflected in Tribal Policies and Procedures Manual.

Other Skills and Abilities:

Required:

This is a required driver position.

Must possess a valid Wisconsin driver’s license and meet eligibility for tribal insurance.

Knowledge of physical fitness programs and ability to organize team sports is required.

Background Check: This position is contingent on ability to pass background check required. This position required for eligibility a required driver check, to meet eligibility for tribal insurance. Caregiver background check is also required when working with children and/or elderly.

Language Skills:

Must have good oral and written communication skills including the ability to write reports, effectively present information and respond to questions from youth and parents.

Reasoning Ability:

Experience working with children and youth in an educational setting

Employees of the Bad River Health and Wellness Center must abide by the Bad River Health and Wellness Center Employee Health Policy. Employees must show proof of Tdap immunization or Pertussis booster, MMR and will receive TB test if employee has not had test within 1 year upon hire.
